No. There is no annual deductible for covered services.

The plan provides up to $2,000 in covered dental benefits per member each calendar year (January 1–December 31).

While many preventive and basic services are covered at 100%, major restorative services are covered at 80%, and orthodontics for eligible dependents is covered at 50%. Coverage is also subject to the plan's annual and lifetime maximums. For complete information, including limitations, exclusions and benefit details, please refer to the United Concordia Certificate of Coverage.
The plan pays 100% for services, including:
- Dental exams
- Routine cleanings
- X-rays
- Fluoride treatments
- Sealants
- Emergency palliative treatment
- Space maintainers
- Fillings
- Root canals (endodontics)
- Periodontal treatment
- Crown, bridge and denture repairs
- Simple and complex extractions
- Oral surgery
- General anesthesia (when covered)
Major services (Class III), including crowns, inlays, onlays, bridges and dentures, are covered at 80%.

Yes. Orthodontic treatment is covered at 50% for eligible dependent children under age 19. The plan includes a $1,000 lifetime orthodontic maximum.

- Unmarried dependent children are covered through age 19.
- Unmarried dependent students are covered through age 26.

The plan reimburses services based on its Maximum Allowable Charge (MAC). Out-of-network dentists may charge more than the plan allows, and you may be responsible for paying the difference (known as balance billing).
